New Patients
Intake for new patients is currently closed. Please return to this webpage at a later date to check if new patients are being accepted.
If you have already submitted an intake form, please allow several weeks for our office to respond as we are currently processing a large number of forms. Voicemails left to inquire about intake status cannot be returned due to an overwhelming number of calls.
Please note that Family Medicine services are only provided at our London location.
